Two people killed in Richland County UTV accident

Aug. 29, 2025

CAZENOVIA, Wis. — Two people were killed Thursday evening in a collision between a pickup truck and a utility terrain vehicle (UTV) in Richland County, authorities said.

The crash occurred around 5:05 p.m. at the intersection of County Highway CC and Hanko Drive, according to the Richland County Sheriff’s Office.

James Koelker, 80, and Sandra Koelker, 79, both of Cuba City, were traveling south in a UTV when they were struck by a westbound pickup truck driven by Kerry McBain, 39, of Cazenovia. A juvenile passenger was also in McBain’s vehicle.

Witnesses told investigators that Koelker had stopped at the stop sign on Hanko Drive but then proceeded into the intersection, failing to yield to the oncoming truck. The impact caused the UTV to roll down the roadway. Both Koelkers were found unresponsive and later pronounced dead at the scene despite life-saving efforts by emergency responders.

Cazenovia Fire and EMS, Reedsburg Paramedics, UW Medflight, and the Richland County Coroner’s Office responded to the scene. The Wisconsin Department of Natural Resources and the Sauk County Sheriff’s Office Accident Reconstruction Team are assisting with the investigation.

The crash remains under investigation.
